1. About Vamps On Wheels LLC

Vamps On Wheels LLC is a mobile diagnostic support company providing mobile and on-site specimen collection and related services.

Services may include, but are not limited to:

• Mobile blood collection
• Specialty laboratory kit collection
• Provider-requested phlebotomy support
• DNA specimen collection
• Gender reveal blood collection
• DOT urine specimen collection
• Non-DOT drug testing specimen collection
• Other authorized drug testing collection services
• Specimen pickup and courier/logistics support
• Facility and organizational collection services
• Other related mobile diagnostic support services

Available services may change periodically.

2. Scope of Services

Vamps On Wheels LLC primarily provides specimen collection and related support services.

Unless specifically stated otherwise, Vamps On Wheels LLC does not independently diagnose medical conditions, prescribe medication, interpret laboratory results, or replace care provided by a licensed physician or other qualified healthcare professional.

Certain laboratory testing requires a valid order, laboratory requisition, collection kit, authorization, or other documentation before collection can be performed.

Clients are responsible for ensuring required documentation is available when applicable.

3. Laboratory Testing and Results

Vamps On Wheels LLC may collect specimens that are subsequently transported or submitted to an independent laboratory or other authorized testing entity.

Unless expressly stated otherwise, Vamps On Wheels LLC does not perform the laboratory analysis of specimens.

Testing, processing, result reporting, turnaround times, specimen acceptance, and laboratory requirements may be controlled by the laboratory or testing provider.

Vamps On Wheels LLC cannot guarantee a specific result turnaround time when testing is performed by an independent laboratory.

Estimated turnaround times are informational and may be affected by laboratory processing, shipping, specimen requirements, holidays, weekends, weather, or other circumstances outside our control.

When applicable, laboratory results may be provided directly by the ordering provider, laboratory, testing company, or other authorized entity.

4. Appointment Requests and Confirmation

Submitting an appointment or service request through our website does not automatically guarantee an appointment.

Appointments are subject to availability, service area, staffing, required documentation, specimen requirements, travel considerations, and other operational factors.

An appointment is considered confirmed only after Vamps On Wheels LLC provides confirmation through an authorized communication method.

Clients are responsible for providing accurate contact information and monitoring communications regarding their appointment.

5. Mobile Service Area and Travel

Vamps On Wheels LLC provides mobile services within designated service areas and may accept appointments outside standard service areas when availability permits.

Travel or mileage fees may apply based on the client's location, service type, distance traveled, contractual arrangement, or other applicable factors.

Any applicable travel or mileage charge will be communicated before service whenever reasonably possible.

Service availability in a particular city, county, or location is not guaranteed.

6. Same-Day and After-Hours Services

Same-day, evening, weekend, holiday, early-morning, or other after-hours appointments may be available depending on scheduling and staffing.

Additional convenience or after-hours fees may apply.

Payment of an additional fee does not guarantee availability unless the appointment has been confirmed by Vamps On Wheels LLC.

7. Client Responsibilities

Clients are responsible for:

• Providing accurate information
• Providing a safe and reasonable location for mobile service
• Being available at the confirmed appointment time
• Following applicable preparation instructions
• Having required identification available
• Providing required laboratory orders, requisitions, collection kits, or documentation when applicable
• Informing Vamps On Wheels LLC of relevant collection requirements provided by their laboratory or healthcare provider
• Providing complete and accurate information required for chain-of-custody procedures when applicable

Failure to meet applicable requirements may result in the appointment being delayed, rescheduled, or canceled.

8. Appointment Preparation

Some laboratory collections may require fasting, timed collection, medication-related instructions, hydration, or other preparation.

Preparation requirements generally originate from the ordering healthcare provider or laboratory.

Vamps On Wheels LLC may communicate preparation instructions provided by another authorized party but does not independently prescribe medical preparation requirements.

Clients should contact their healthcare provider or laboratory if they have questions regarding medical preparation for testing.

9. Blood Collection and Collection Attempts

Vamps On Wheels LLC uses reasonable professional efforts to successfully collect specimens.

However, successful specimen collection cannot be guaranteed.

Factors including dehydration, vein condition, medical history, specimen requirements, client tolerance, and other circumstances may affect collection.

For safety and quality reasons, Vamps On Wheels LLC may limit collection attempts during an appointment.

If a specimen cannot safely or successfully be collected, the appointment may need to be discontinued or rescheduled.

Fees associated with unsuccessful collections, redraws, or repeat visits may vary based on the circumstances, contractual arrangement, service requested, and applicable policies.

10. Specimen Integrity and Recollection

Vamps On Wheels LLC follows applicable collection, labeling, handling, and transportation procedures within the scope of the services provided.

However, specimens may occasionally require recollection because of circumstances including:

• Laboratory rejection
• Insufficient specimen quantity
• Clotting
• Hemolysis
• Improper or changed laboratory requirements
• Shipping or transportation issues
• Testing facility requirements
• Circumstances outside the reasonable control of Vamps On Wheels LLC

Whether an additional collection or travel fee applies will depend on the reason for recollection and the applicable service arrangement.

11. Specialty Laboratory Kits

Clients using specialty laboratory kits are responsible for ensuring that required kits, laboratory paperwork, orders, and materials supplied by the testing company are available when required.

Vamps On Wheels LLC is not responsible for errors, delays, missing materials, expired kits, incorrect kit contents, laboratory processing, or testing requirements controlled by an independent testing company.

12. DNA Collection Services

DNA collection services may involve informational, legal, relationship, or other authorized testing depending on the service selected.

Requirements vary depending on the testing purpose.

Legal or chain-of-custody DNA testing may require identification, documentation, witnessed collection, photographs, signatures, or other procedures established by the testing laboratory.

Vamps On Wheels LLC provides collection services according to applicable instructions but does not determine DNA testing results.

A collection appointment does not guarantee a particular test outcome or laboratory processing time.

13. Gender Reveal Collection Services

Gender reveal services involve specimen collection for testing performed by an applicable laboratory or testing provider.

Vamps On Wheels LLC does not independently determine fetal sex or perform the laboratory analysis unless expressly stated otherwise.

Eligibility, gestational-age requirements, accuracy representations, processing times, and result delivery are governed by the applicable testing provider.

Clients are responsible for meeting the testing provider's eligibility requirements.

14. DOT Drug Testing Collections

DOT-regulated specimen collections are performed according to applicable collection procedures and requirements.

Vamps On Wheels LLC serves as a collection provider and does not control laboratory analysis, Medical Review Officer determinations, employer decisions, or federal agency determinations.

Individuals and organizations requesting DOT testing are responsible for ensuring that the correct test type and applicable authority or agency requirements have been identified.

15. Non-DOT Drug Testing Collections

Non-DOT drug testing services may be provided to employers, organizations, facilities, or other authorized clients.

Testing requirements, panels, laboratory analysis, employer policies, and result interpretation may be controlled by the requesting organization, laboratory, Medical Review Officer, or other authorized party.

Vamps On Wheels LLC does not make employment decisions based on drug testing results.

16. Chain of Custody

Certain collections require formal chain-of-custody procedures.

When chain of custody is required, clients must comply with applicable identification, documentation, signature, specimen-security, and collection requirements.

Vamps On Wheels LLC reserves the right to discontinue or decline a collection when required chain-of-custody procedures cannot be completed properly.

17. Courier and Specimen Transportation Services

When Vamps On Wheels LLC provides specimen pickup, delivery, or courier services, specimens are transported according to applicable handling requirements within the scope of the service requested.

Pickup and delivery times may be affected by traffic, weather, laboratory operating hours, access restrictions, emergencies, or other circumstances beyond our reasonable control.

Vamps On Wheels LLC does not control laboratory processing after a specimen has been properly transferred to the receiving laboratory or authorized recipient.

18. Pricing

Current service pricing may be displayed on our website, provided in a quote, included in an agreement, or communicated directly to the client.

Prices may vary based on:

• Service requested
• Location
• Travel distance
• Number of individuals
• Number or type of specimens
• Supplies required
• Appointment timing
• Same-day or after-hours requests
• Contractual or organizational pricing
• Other service-specific requirements

Vamps On Wheels LLC reserves the right to update pricing.

Pricing changes will not retroactively alter the agreed price of a previously confirmed and paid service unless the scope of the requested service changes.

19. Payment

Payment requirements vary by service.

Payment may be required at booking, before collection, at the time of service, or according to the terms of a written organizational or vendor agreement.

Clients are responsible for charges associated with the services they authorize.

Additional services requested during an appointment may result in additional charges when applicable.

20. Insurance and Reimbursement

Unless specifically stated otherwise for a particular service, clients should not assume that Vamps On Wheels LLC will bill their health insurance plan.

A laboratory, healthcare provider, or other organization may separately bill insurance for testing or related services.

Charges from an independent laboratory, healthcare provider, testing company, or other entity are separate from Vamps On Wheels LLC's collection or mobile service fees unless specifically stated otherwise.

Clients are responsible for verifying insurance coverage directly with their insurer or applicable provider.

21. Cancellations and Rescheduling

Clients should notify Vamps On Wheels LLC as soon as possible if an appointment must be canceled or rescheduled.

Cancellation, rescheduling, travel, missed appointment, or late-cancellation fees may apply depending on the service and how much notice is provided.

If a collector has already begun traveling to the appointment location, applicable travel or service charges may remain due.

Specific cancellation requirements communicated during booking or contained in a separate service agreement will control when applicable.

22. Late or Missed Appointments

Clients should be available at the agreed appointment location at the scheduled time.

Significant delays may affect subsequent appointments and may require rescheduling.

If the client is unavailable, cannot be contacted, refuses service after arrival, or cannot complete the appointment because required documentation or materials are unavailable, applicable service or travel fees may remain due.

23. Refunds

Because Vamps On Wheels LLC provides mobile, appointment-based professional services and may incur travel, scheduling, supply, and administrative costs before an appointment, payments are not automatically refundable solely because a client changes their mind.

Refund eligibility will depend on the circumstances, services already performed, expenses incurred, applicable cancellation policy, and any separate written agreement.

If Vamps On Wheels LLC cancels an appointment and cannot reasonably reschedule the service, amounts paid directly to Vamps On Wheels LLC for services not performed may be eligible for refund.

Fees charged independently by laboratories, testing providers, payment processors, or other third parties are subject to those entities' policies.

24. Right to Decline or Discontinue Service

Vamps On Wheels LLC reserves the right to decline, discontinue, or reschedule a service when reasonably necessary because of:

• Unsafe conditions
• Threatening, abusive, discriminatory, or inappropriate behavior
• Collection requirements that cannot safely be completed
• Missing required documentation
• Improper testing materials
• Suspected specimen tampering
• Chain-of-custody concerns
• Conditions outside the scope of services
• Nonpayment
• Circumstances that could compromise client, collector, specimen, or public safety

This provision does not permit unlawful discrimination.

25. Emergency Medical Situations

Vamps On Wheels LLC is not an emergency medical service.

Our website, phone number, appointment system, and mobile collection services should not be used to request emergency medical care.

If you are experiencing a medical emergency, call 911 or seek immediate emergency medical assistance.

26. Privacy

Use of our website and services is also subject to the Vamps On Wheels LLC Privacy Policy.
